1. He is the recipient of many honours, in India and abroad in recognition and appreciation of his Service to humanity. Honorary citizenship is confirmed on Dr. Modi by the mayor of Spartanburg, U.S.A.
2. Karnataka & Mysore Universities has conferred Honorary degrees for his Yeoman Services.
3. Conducted more than 5 lakh Eye Operations & has done 833 Eye Operations in a day rendering intensive free Eye relief Services since 1943, & has been recorded in the Guinness Book of World records.
4. Associated with many National and International Service Organisation. And also won Government of Karnataka Rajyothsava Award & Smt. Indira Gandhi National Unity Award.
5. Awarded "Padmashri" in 1956 and "Padrna Bhushan" in 1968 and won many National ·and International Awards.
6. Constructed a Free Eye Hospital in Davanagere and Mobile Eye Clinic. He has been called as "Netradani" by the people of India.
7. His moto is Services to Humanity.
   
8. He has been nominated to Kamataka Legislative Council on 9th August, 1990.
